On this page, you will find below the online request form that needs to be filled out, which includes personal information such as name, contact number, and email address.

Individuals must also attach a clear picture of the certificate they wish to verify, in jpeg or pdf format. Once the form and the attachment have been submitted, the request will be reviewed by the our staff and the certificate's authenticity will be verified by checking its validity against our database.

Once the certificate has been verified, individuals will be notified through the provided contact information. It is important to note that our TESDA certificate verification service is only available for certificates issued by this instituion. This online verification service is for a convenient and efficient way for individuals to verify their certificates issued by this instituion, without the need to physically visit the TESDA Provincial Training Center in Zamboanga Sibugay.
